Chapter 155 Second Year Medical Resident

Haruto had just started his second year as a resident. For the past few months, he had rotated through pediatrics, obstetrics and gynecology, and psychiatry—each rotation lasting about a month. Now, he was in the final phase of his training: community health and medicine.
Community health covered a lot, but the clearest example was working in places with doctor shortages—remote towns or isolated islands.
Haruto was currently assigned to a small clinic on a remote island. The clinic was managed by several university hospitals that sent doctors on a monthly rotation.
Naturally, this meant living away from the city.
”What does it mean to be unrecognized?!” Haruto snapped into the phone.
”As I said, you’ll stay at this clinic until your training ends,” came the firm reply from the management division at Chiba University Hospital, which oversaw resident training.
”I just can’t accept that… pfft…” Haruto’s voice cracked with frustration.
He had applied to train in emergency medicine for about eight months of his second year, but his request was denied. Worse, he wasn’t allowed to switch to any other department and had to finish his residency here in community health.
”Damn it! It’s definitely the director’s doing,” Haruto muttered, convinced the hospital director was behind this decision.
His suspicion came from what happened just before his emergency medicine rotation was supposed to end.
An internal announcement had blared through the hospital’s speakers:
Code Blue. All related personnel, please report to the obstetrics and gynecology lobby on the third floor.
Haruto, who happened to be nearby, sprinted faster than anyone else to the scene.
A Code Blue was a hospital emergency call signaling a sudden critical condition. It summoned all available doctors and nurses—no matter their specialty—because the cause was unknown and every hand was needed.
Other codes were used too:
* Code White: For verbal threats or violence inside the hospital.
* Code Orange: For disaster situations.
When Haruto arrived, a nurse was performing chest compressions and artificial respiration on a woman who had collapsed.
”No medical tools, huh… it’s a race against time until we can get to the secondary procedures,” Haruto thought.
This was bad—it looked like an amniotic fluid embolism.
Amniotic fluid embolism occurs when amniotic fluid, fetal cells, or tissue enters the mother’s bloodstream, causing a severe reaction.
More staff arrived quickly, and Haruto took charge.
”It’s amniotic fluid embolism! Check inside!” he ordered.
”The head is already out,” the nurse reported.
This condition usually happens late in labor or right after delivery, so although the woman’s due date was still days away, her abdominal pain was actually labor starting early.
”No time to move her. We have to deliver the baby right now! Get the sheets, surround her!” Haruto commanded.
””””Yes!””””
The team split into two: one for maternal resuscitation, the other for delivering the baby.
Haruto continued chest compressions, anxiety gnawing at him.
Amniotic fluid embolism was deadly—the maternal mortality rate ranged from 37% to 80%.
