 Doctor Helicopter

 In his past life, Haruto had first learned about doctor helicopters when they were still a rare sight. Now, in this world, they had become an everyday part of emergency medicine.


 Back then, when Haruto died, Japan had just begun rolling out hospital helicopters capable of transporting critical patients. But now, thanks to advances in AI and changes in the law, even medical residents could board these helicopters as part of their training.


 Normally, a doctor helicopter crew consists of four members: a pilot, a mechanic, a flight doctor, and a flight nurse. The cabin, however, can only fit two patients at most. That meant if a resident boarded, only one patient could be transported, making it inefficient for many hospitals to include trainees on emergency flights.


 But with the development of highly advanced AI and new legal regulations, hospitals began operating with just one pilot. The seat that used to belong to the mechanic was now open—and that’s where residents like Haruto would sit. It was a chance to train new doctors in real emergency settings.


 One day, as Haruto was managing routine emergency transports, the dedicated doctor helicopter PHS started to ring. Without hesitation, he rushed to the special operations room.


 ”Are you ready, Ichijou?”


 ”Yes, I’m ready!”


 In the room, several deployment bags were already prepared. Grabbing the necessary gear, Haruto followed the staff toward the helipad.


Whoooosh—


 The roar of the engine echoed the moment they stepped outside. The wind whipped at their coats as they ran. With each step closer, the pressure intensified.


 (The wind’s stronger than I thought!)


 The massive blades spun overhead. Logically, he knew he wouldn’t get hit, even if he stood directly beneath them. But the instinct to duck was overwhelming—his body tensed, and he crouched low as he moved forward.


 Once inside, Haruto quickly put on the communication headset. Without it, the noise made it impossible to hear anything.


 ”Wow… We’re lifting off.”


 He couldn’t stop the words from escaping his lips. The flight doctor and nurse sitting beside him gave warm, knowing smiles. It was probably obvious this was his first time riding in one.


 Suddenly, a voice came through the headset.


 ”A female in her sixties. Complaining of back pain, collapsed shortly after.”


 Haruto’s mind sharpened immediately. The time for awe was over.


 ”Prepare the ultrasound,” he said, steady and clear.


 He sifted through the information as it came in, narrowing down the possibilities in his mind. What were the likely causes? What actions would be needed once they landed?


 About fifteen minutes after liftoff, the helicopter touched down on a high school athletic field. A crowd of students peeked out from the windows of the building nearby, watching the scene unfold.


 Haruto followed the team into the waiting ambulance. The nurse leaned over to the woman lying on the stretcher.


 ”Can you hear me? Can you understand me?”


 Haruto wasted no time. He gently placed the ultrasound probe on the patient’s back, eyes scanning the monitor. He already had a suspicion: aortic dissection.


 The scan was also being displayed in real-time back at the hospital. Even though the diagnosis was nearly certain, confirming it was still necessary.


 *Aortic dissection occurs when a tear forms in the inner wall of the aorta. Blood flows into the wall itself, creating a second, false passage. It often strikes without warning, causing intense chest or back pain.*


 (It’s hard to confirm with just the ultrasound…)


 Still, Haruto pointed at the image and opened a line to the hospital.


 ”Chiba University, are you watching?”


 ”This is Chiba University. Go ahead.”


 ”We suspect a dissection. We’re considering transport. Do you agree?”


 ”Understood. We will accept the patient.”


 Without delay, the woman was moved onto the helicopter, and they lifted off once more—this time, bound for Chiba University Hospital.


 Haruto let out a quiet breath of relief. Everything was going according to plan.


 But then—


 ”Ichijou-sensei… I can’t feel a pulse.”
